Steven D. Nathan, MD, who holds the Schar Chair of advanced lung disease and lung transplant at Inova Schar Heart and Vascular, told the American Journal of Managed Care that primary‑care clinicians must stay alert to early indicators of the condition.
“What separates the good from the great primary care providers are those who recognize a difference in those with interstitial lung disease,” Nathan said. “The clues in particular are when listening to the lungs, listen for crackles at the bases, typical Velcro crackles, or what is described in idiopathic pulmonary fibrosis, and it sounds like Velcro getting pulled apart.”
Beyond the distinctive crackles, shortness of breath remains the most common presenting complaint. He recommends that any patient with unexplained dyspnea undergo pulmonary function testing to help rule out or confirm interstitial involvement.
Referral to specialty centers when uncertainty remains
When primary clinicians are unsure, the doctor advises prompt referral to tertiary care facilities equipped to perform high‑resolution imaging and multidisciplinary evaluation.

“It takes an astute physician or clinician to recognize that this is interstitial lung disease and not one of the other more common conditions,” he said during a discussion at the Washington, DC, Pop Health Roundtable hosted by the same journal.
At those centers, specialists can coordinate bronchoscopy, lung biopsy, and detailed imaging to reach a definitive diagnosis, reducing the risk of prolonged treatment delays.
While the need for specialist input is clear, the reality of busy primary‑care settings means that many patients still slip through the cracks.
Expanding continuing‑education programs on lung auscultation could help clinicians more reliably identify the Velcro‑type crackles that signal interstitial disease.
